The patient motivation programs developed by MLS are designed to encourage patients with a chronic disease to manage actively their condition. Depending on their clinical and psychological condition, patients enroll either follow-up programs - allowing to identify remission and relapse phases or motivation programs. Motivation programs are suited to patients needing to adapt their day-to-day behavior and life habits to their disease.

Patient-centric programs include a set of objectives depending on the disease profile and, for each objective, a set of adapted questions for patients and/or for healthcare and social services providers to respond to. Coordination and information sharing mechanisms and tools are the cornerstones of the program:
Collect in real-time and on-the-field medical, HE and OR, QOL and social information
Enhance collaborative work processes among healthcare providers and possibly the patients
Structure dialogues based on patient protocol defined by healthcare providers
Centralize coordination of the programs
Analyze and dispatch health-data collected and notifications, based on pre-defined workflow. |
