The interRAI Community Health (CHA) Assessment System was developed in response to user requests
for a modular instrument that could efficiently assess the well elderly individual and identify those
persons who merit further assessment in order to prevent or stabilize early functional or health decline.
The CHA instrument comprises a brief basic assessment and four supplements: Functional (CHA-FS),
Mental Health (CHA-MH), Assisted Living (CHA-AL) and Deafblind (CHA-Db). The target population for
the CHA instrument is adults living in the community and a variety of residential care settings.
The interRAI Long-Term Care Facilities Assessment System (interRAI LTCF) enables a comprehensive,
standardized evaluation of the needs, strengths and preferences of persons receiving short-term postacute
care in skilled nursing facilities as well as persons living in chronic care and nursing home
institutional settings.
The interRAI Home Care Assessment System (HC) has been designed to be a user-friendly, reliable,
person-centred assessment system that informs and guides comprehensive care and service planning in
community-based settings. It focuses on the person’s functioning and quality of life by assessing needs,
strengths and preferences, and facilitates referrals when appropriate.
The interRAI Contact Assessment (interRAI CA) Screener provides information to support the home care
intake process for emergency departments and intake from Community/Hospitals.
